The nurses, in collaboration with the Ontario Health at Home (OHaH), will closely monitor your parent's ability to manage safely at home. Further services and/or safety equipment may be put in place, or your family may be asked to help plan for alternative living arrangements for your parent. Whatever the outcome, our job is to help guide and support you through any transition.

In being fiscally mindful, daily visits may not be carried out long term. However, there are many strategies to assist your parent with their health care needs including dosettes, safety equipment or protective surfaces. Our nurses are committed to working closely with the physician to optimize health and independence.

You may still receive services at home if you do not have a family physician. However, you may be referred to a walk-in clinic or emergency department for further assessment, or to obtain a physician's order if your nurse feels this is necessary.
