Health Teaching is any combination of learning experiences designed to help individuals and communities improve their health, by increasing their knowledge or influencing their attitudes.

The purpose of health teaching is to positively influence the health behavior of individuals and communities, as well as the living and working conditions that influence their health. The goal of health teaching at Partners in Community Nursing is to improve the health status of individuals, and to promote health through education-driven behaviors and related activities.

  • Our nurses will work with you and your caregiver to provide the knowledge and skills you will need to manage your health care independently. This may include learning to give your own injection, and the nurse will support you through the process.

  • Learning to manage your own care will give you the opportunity to live more independently. It means having the skills and confidence to take charge of your medical needs.

  • Nursing Care:

    • Available 24 hours a day, 7 days a week

    Administrative Support:

    • Monday-Friday, 7:00 AM - 11:00 PM

    • Saturday, Sunday & Holidays: 8:30 AM - 8:30 PM

    After-hours support:

    • 24/7 on-call nurse available for urgent concerns

  • Most services are funded at no cost through Ontario Health at Home (OHaH) for eligible clients. Inquire with your doctor today.

    Self Referral:

    Anyone can make a referral to us on your behalf and with your consent – a family doctor, friend, family member, caregiver, neighbour and even you, yourself.

    We also accept private insurance and private payment arrangements. Contact us to discuss funding options for your specific care needs.
