We recognize that health needs often change at this time of life. Our focus is on health maintenance and optimizing independence. Regular assessments will identify risks to health and well-being, and a plan of care will be established, including links to other services according to your needs.

Frequently Asked Questions

  • An assessment can be completed over the phone by contacting the Ontario Health at Home (OHaH) at 1-800-263-3877. Services designed to care for your parent at home may include but not be limited to: personal care, medication management, safety assessments and end of life care. Private insurance may augment to cover the cost of nursing and support services, or if you have the resources, you may pay privately.

  • The nurses, in collaboration with the Ontario Health at Home (OHaH), will closely monitor your parent's ability to manage safely at home. Further services and/or safety equipment may be put in place, or your family may be asked to help plan for alternative living arrangements for your parent. Whatever the outcome, our job is to help guide and support you through any transition.

  • In being fiscally mindful, daily visits may not be carried out long term. However, there are many strategies to assist your parent with their health care needs including dosettes, safety equipment or protective surfaces. Our nurses are committed to working closely with the physician to optimize health and independence.

  • You may still receive services at home if you do not have a family physician. However, you may be referred to a walk-in clinic or emergency department for further assessment, or to obtain a physician's order if your nurse feels this is necessary.

    • Available 24 hours a day, 7 days a week

    Administrative Support:

    • Monday-Friday, 7:00 AM - 11:00 PM

    • Saturday, Sunday & Holidays: 8:30 AM - 8:30 PM

    After-hours support:

    • 24/7 on-call nurse available for urgent concerns

  • Most services are funded at no cost through Ontario Health at Home (OHaH) for eligible clients. Inquire with your doctor today.

    Self Referral:

    Anyone can make a referral to us on your behalf and with your consent – a family doctor, friend, family member, caregiver, neighbour and even you, yourself.
