Palliative Care is provided to any client or family diagnosed with a life-threatening illness with the specific aim to relieve suffering, and to improve one's quality of living and dying.

Partners in Community Nursing has palliative care consultants prepared with Comprehensive Advanced Hospice Palliative Care Education (CAPCE). We are committed to respecting your individual values and beliefs. Our nurses work closely with clients and their families to address physical, psychological, spiritual, and social issues, and to assist them with their hopes and fears.

We work closely with physicians to obtain optimal pain control, as well as alleviating other commonly related physical symptoms such as nausea, vomiting, constipation, shortness of breath and maintenance of skin integrity.

  • The level of palliative care available to clients at home depends on where they live. Certain geographical areas have palliative care physicians that can make home visits. Nursing care and personal support care in the home are available, and the amount of time these healthcare professionals can spend in the home is determined by the needs of the person living with a life-threatening illness.

  • Partners in Community Nursing will provide health teaching to you and your family on various pharmacological, as well as non-medicinal methods to achieve and maintain comfort. In addition, we have a nurse on call 24 hours a day to assist you in obtaining maximum pain control when necessary.

  • No, as long as the medication is taken to relieve physical pain, addiction should not occur.

    Administrative Support:

    After-hours support:

    • 24/7 on-call nurse available for urgent concerns

  • Most services are funded at no cost through Ontario Health at Home (OHaH) for eligible clients. Inquire with your doctor today.

    Self Referral:

    Anyone can make a referral to us on your behalf and with your consent – a family doctor, friend, family member, caregiver, neighbour and even you, yourself.
