Supporting Your Recovery at Home

Post operative care at home is increasingly important as hospital stays become shorter. Your Partners in Community Nursing nurse's role is to assess overall health status and promote healing.

Wound care is provided as specified by your surgeon. A plan of care will be developed by your nurse in cooperation with the Ontario Health at Home (OHaH). If your wound is not progressing as anticipated, a wound care specialist will be consulted to ensure optimal healing. Your nurse will also monitor pain, nutrition, as well as other factors that may impact healing, and follow up will be made with your surgeon when appropriate.

Frequently Asked Questions

  • The frequency of nursing visits will be based on best practice and in collaboration with the Ontario Health at Home (OHaH). If appropriate, we will teach a support person or family member to provide your wound care between nursing visits.

  • Medical supplies such as gauze, tape or specialized products are determined by your wound, ordered by your nurse, and provided by the Ontario Health at Home (OHaH).

  • Your nurse will review with you what can be expected during your post-op recovery period, and changes that may require a call for advice. Partners in Community Nursing has a nurse available 24 hours/day, 7 days/week via telephone if you have any concerns such as increased pain, wound drainage, bleeding or fever.
